The first upgrade - allowing the player to continue receiving cookies even when they are not clicking - is the start of a large number of upgrades. When the player has collected enough cookies, they can upgrade to things like idle clickers to receive cookies automatically.

Clicking on the cookie will grant the player a cookie. That is the case with Cookie Clicker, which essentially only requires the player to be able to click their mouse.

Idle - or clicker - games typically require no skill when it comes to gameplay.
